Our mission is to organize and support a continuum of post-acute healthcare providers with the resources to manage and improve quality of care through improved access, coordinated care and improved efficiency.  We are demonstrating value to managed care organizations (MCO) and accountable care organizations (ACO) through efforts to develop and adhere to uniform practice standards in our efforts to create economic benefits for all parties.
